Long-Term Care Program
Don’t let the name fool you! The Community Paramedicine Long-Term Care Program (CPLTC) is funded by the Ontario Ministry of Long-Term Care, but is geared primarily towards supporting older adults in the community who are awaiting Long-Term Care Home placement and/or frail older adults who wish to remain in their own home but may be at higher risk of hospitalizations. The paramedics work alongside other health care professionals and community agencies to provide accessible, non-emergency services. The Community Paramedics use a combination of scheduled and on-demand home visits to help prevent unnecessary 911 calls and hospital admissions. Services are personalized to the client’s needs and may include mobility support, medication review, chronic illness management, assessment and treatment of non-emergency medical concerns, and referrals to other community agencies. This program is available 24/7.
Eligibility
- 65 years of age or older and on the waitlist for long-term care homes or are soon to be eligible for this waitlist
OR
- 65 years of age or older and considered at high-risk of failing at home, including those who live alone, have a high-risk of falls, have a complex medical history, and/or mild to severe cognitive impairment
*For those living in a retirement home, they must be currently on the Long-Term Care Home waitlist to meet program eligibility.
