High Intensity Supports at Home (HISH)

The HISH Program provides short-term, frequent supports to clients with complex needs. The paramedics on this team are specially trained to support those with complex care needs, by providing in home supports and working closely with the client’s health care team. Services may include caregiver support, remote monitoring, early assessment/treatment, and transition support.

Eligibility

To provide the level of support needed, the High Intensity Supports at Home program only takes on a very limited number of clients, and eligibility to the program is determined on a case by case basis.
