Health Promotion Program

The Community Paramedic Health Promotion program aims to support at-risk individuals before their condition deteriorates to require emergency or acute care. It focuses on clients who are at risk of falling through the cracks in the health care system for a variety of socio-economic or circumstantial reasons. The program seeks to provide early intervention and referrals to appropriate services, in addition to health teaching and wellness clinics. 

Emergency Shelter Clinics

This initiative aims to meet the health care needs of vulnerable populations by connecting unhoused individuals with the health care system. Paramedics host weekly wellness clinics at emergency shelters to provide health assessments and minor treatments to clients, as well as referrals to appropriate community agencies, health programs, or support services.  

Community Health Clinics

Community Health Clinics are designed to prevent illness and injury in older or at-risk adults to reduce hospitalizations and improve their quality of life. Paramedics hold bi-weekly drop-in clinics at designated housing complexes where they assess residents’ health and provide teaching, support chronic disease management, and offer assistance through referrals to other agencies. They can also liaise with existing care teams on behalf of the client.
