Nomination Criteria

Awards are open to individual volunteers and non-profit volunteer groups, such as clubs, societies or associations, for their contributions through services, programs, projects and/or events delivered within the City. 

The nomination form must be completed by the nominator. Please research and clearly describe the volunteer’s accomplishments and ensure all sections of the form are completed. Note: If the nomination is successful, a short summary of the nominee’s achievements will be prepared from the nomination form and read during the awards ceremony. 

The volunteer’s efforts should help to contribute to creating a healthy community (i.e., civic engagement, environment, economic growth and human health and well-being). 

Please provide as much detail as possible about the nominee’s achievements, as this information helps the selection committee assess eligibility. 

Nominees must not have received a Civic Award within the past five years. A list of previous award recipients is available below.
