How to Nominate a Volunteer or Group of Volunteers

Nomination forms are available during the nomination period only: 

• Online: Complete the nomination form electronically 

• In person: Paper copies are available at: 
  • One-Stop Services at Tom Davies Square, 200 Brady Street, Sudbury, ON P3E 3L9 or
  • Any Citizen Service Centre.

         Deadline for submission: 
         Monday, March 2, 2026, at 4:30 p.m.
